PLEP:

In his evaluation dated 10-18-XX, Sean was identified as Other Health Disability because of his inability to complete educational tasks in routine timelines due to his ADD diagnosis. At this time, Sean does not have a system for tracking assignments to completion and turned into his teachers. His parents state that Sean never knows what he has for homework or when tests are coming. They have been checking with his sixth grade teachers about assignments, but feel Sean is old enough to start taking care of that himself. He currently has 5 or more assignments missing in each of his seven classes. He has a D in four classes and is failing the other three. Sean is able to do the work academically, but his lack of an organizational system is greatly impacting his school success.

Goal:

Sean will improve his organizational skills from a non-existent system (35 or more missing assignments) to one that improves his assignment completion to less than 18 assignments missing. 

Objectives:

After trying three different organization systems each for at least one week, Sean will identify at least 2 pros and 2 cons for each system as measured by a summary chart.

After choosing the system he feels is best for him, Sean will apply it daily and will turn in completed assignments on time, with adult assistance when Sean requests, so that less than 18 assignments are missing as measured by classroom teacher gradebooks.
